The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase the visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across communities and healthcare systems in partnership with community, faith-based, and healthcare organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve the health and well-being of New Yorkers.

 

This position is grant-funded.

 

POSITION OVERVIEW

The Community Outreach and Data Coordinator will be responsible for planning and executing outreach and data collection efforts of a grant funded initiative that engages community residents in local businesses to address hypertension and social determinants of health.  The qualified candidate will be responsible for participating in community outreach, data surveillance, research, and related program evaluation activities. The Community Outreach and Data Coordinator will also be responsible for using statistical tools to analyze data that they have collected, interpreting trends, and reporting progress to the Director of Community Outreach and Senior Director of Research, Evaluation, and Planning. 

 

RESPONSIBILITIES

  • Engaging with community partners to promote health services, resources, and education.
  • Develop and maintain relationships with a broad variety of community partners.
  • Representing the Bureau of Bronx Neighborhood Health at health fairs, meetings, and other external events occurring throughout the community.
  • Coordinating chronic disease programming in the South Bronx.
  • Delivering presentations and workshops to diverse audiences throughout the community.
  • Maintaining and reporting data that demonstrate community activities and results.
  • Collecting and analyzing program data.
  • Developing tracking systems.
  • Creating reports and visualization.
  • Presenting program data at internal and external meetings.
  • Supporting Bureau-wide programs and initiatives.
  • Supporting Agency-wide emergency response efforts.

 

QUALIFICATIONS

  • Bachelor’s degree and 2-3 years community work experience.
  • Demonstrated capability to lead the implementation of special community-level initiatives.
  • Excellent organizational skills and highly detail-oriented.
  • Excellent oral and written communication skills.
  • Demonstrated ability to establish and maintain working relationships with community groups effectively.
  • Bilingual; Spanish preferred.
  • Knowledge of Bronx local communities and local organization leadership.

 

SALARY 

  • Salary range is $50,000 to $62,000

9:00 am – 5:00 pm
