The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. PROGRAM OVERVIEW

The Bureau of Health Equity Capacity Building (BHECB) seeks to implement collective action strategies to build community power and capacity to address health inequities and the social determinants of health. BHECB develops strategies, activities, and processes that build, strengthen, and maintain the skills and resources necessary to improve health outcomes in NYC's underserved communities. HECB’s core capacity-building programs focus on community engagement, place-based programming, chronic disease prevention, climate change and climate justice, and violence prevention. BHECB is committed to advancing Healthy NYC, New York City’s campaign for healthier, longer lives.

Advancing Care and Equity for Diabetes Prevention and Management, referred to as “ACED”, is a NYC Department of Health and Mental Hygiene (DOHMH) place-based initiative within “high-needs” counties to decrease risk for type 2 diabetes through improved screening for diabetes risk and increase access and completion of community change programs for those at risk; decrease the proportion of people with diabetes that have an A1C>9% and increase early detection of diabetes-related complications through improved self-care practices and quality of care; and advance health equity by addressing the SDOH that impacts priority populations.⁵ Using a collective impact approach, DOHMH will partner with community-based and clinical organizations to expand access to an array of sustainable services to prevent diabetes, foster self-management among people with or at risk for diabetes and implement evidence-based interventions to improve the management of diabetes. Underscoring the role of the SDOH in diabetes prevention and management, services will include interventions to increase screening and referral to programs/services that address the SDOH. DOHMH will also implement and evaluate innovative methods of community and patient outreach and engagement, patient-provider communication, and link health care and community resources among selected populations and neighborhoods.

POSITION OVERVIEW

This role will report to Korin Parrella, Executive Director, Community Resilience and Response Unit, Health Equity Capacity Building. The incumbent will coordinate across the ACED grant team and HECB to align ACED's diabetes prevention/management strategies with HECB's community engagement and chronic disease prevention goals. The incumbent will work to incorporate Healthy NYC's vision for how to improve life expectancy and create a healthier city across unit initiatives by ensuring that Bureau activities and metrics are aligned with HealthyNYC’s strategic goals.

RESPONSIBILITIES

• Support the Bureau with managing programs and deliverables to align and meet ACED goals and objectives to ensure alignment with Healthy NYC goals and objectives, with a focus on diabetes, chronic disease, and the health impacts of climate change on New Yorkers with comorbid conditions.
• Liaise with project leads and other staff overseeing discrete projects in the Bureau’s CRRU /Healthy NYC portfolio.

• Develop, implement, and maintain internal project management workflows for HECB ACED and associated Healthy NYC strategies, activities, and objectives.
• Coordinate across the ACED grant team and HECB to align ACED's diabetes prevention/management strategies with HECB's community engagement and chronic disease prevention goals.
• Directly support liaising with Healthy NYC leadership teams, including attending workgroups, coalition meetings, and other spaces to receive routine updates on Healthy NYC and communicate progress as needed.
• Catalogue and disseminate chronic disease-related resources available to community partners and other external stakeholder groups.
• Develop written documents, including summaries and templates, in support of cross-grant reporting and for presentation to internal and external stakeholders communicating multiple elements of Healthy NYC, diabetes, and chronic disease prevention.
• Other tasks as required.

QUALIFICATIONS

• Bachelor’s level education with 2-3 years of work experience, or Masters’ degree in relevant field (Public Health, Public Administration, etc.) may substitute for work experience

• Strong written and oral communications skills
• Quick learner and able to work in a fast-paced environment
• Experience with federal grant management and/or reporting preferred
• Competent in Microsoft Suite (Outlook, Word, Excel)
• Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities, along with a demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)

SALARY 

Salary range is $60,00 to $62,500
