 The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between the government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

PROGRAM OVERVIEW
 
The NYC Department of Health is seeking a Community Health Worker to provide point-of-care caregiver and family education for pediatric asthma self-management and support. The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers.

 

This position is grant-funded.

 

 RESPONSIBILITIES

 

  • Engage pediatric-patients, caregivers and their families in the pediatric emergency room, the inpatient pediatric floor, and over the phone to assess causes of asthma exacerbation. 
  • Assess baseline knowledge and fill in knowledge gaps using educational materials to describe physiology, medications and triggers for asthma.
  • Provide education about hospital and health department resources to help control asthma and mitigate environmental triggers
  • Provide education on Medication Administration Forms
  • Review asthma action plans
  • Assess for health-related social needs and refer as needed to programs such as,
    • Integrated Pest Management (IPM)
    • Refer to internal or external case management services within health system or health department funded organizations when other issues are identified.
  • Review medication prescriptions to ensure appropriate asthma medication, discuss with ED provider about needed optimization, and address any barriers related to the pharmacy or patient’s insurance.
  • Assist in facilitation of follow-up appointment scheduling with primary care provider, subspecialist, or clinical representative from the office of school health.
  • Document each patient encounter in detail in electronic medical record.
  • Collect and summarize data to monitor patient touches, changes in outcomes
  • Take part in public health detailing initiatives in the South Bronx when needed.
  • Performs other duties as assigned.

 

QUALIFICATIONS

  • High school diploma/ GED or equivalent required
  • Associate degree or Bachelor’s degree in community health or related field preferred
  • Bilingual (English/Spanish or English/French) is required
  • Strong relationship builder with excellent interpersonal and communication skills 
  • Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred 
  • Experience working with marginalized populations preferred
  • Healthcare related experience is a plus 

 

SALARY

$55,000 - $60,000

In - Person

8:00 am– 4:00 pm

Tuesday – Saturday

RESIDENCY REQUIREMENT
