Rehab Case Manager - Per Diem

We are seeking a dedicated and compassionate Case Manager to join our team in our inpatient rehabilitation unit. The Case Manager will play a crucial role in coordinating patient care, ensuring seamless transitions throughout the rehabilitation process.

Responsibilities:

• Conducting comprehensive assessments to develop individualized care plans.
• Collaborating with a multidisciplinary team, including physicians, nurses, therapists, and social workers, to optimize patient outcomes.
• Monitoring patient progress and adjusting care plans as needed.
• Serving as a liaison between patients, families, and healthcare providers to ensure clear communication and support.
• Facilitating discharge planning and connecting patients with appropriate community resources.

Qualifications:

• Bachelor’s degree in nursing, social work, or a related field
• Relevant case management experience in a healthcare setting.
• Strong communication and organizational skills.
• Knowledge of rehabilitation services and discharge planning processes.
