Job Summary: Under the general supervision of the Community Services Division Manager, ensures that the service plan and service delivery for each program participant is integrated, coordinated and monitored, and is designed to support the participant in a manner that helps the participant to achieve the highest possible level of independent functioning. The responsibilities of a case manager shall include the responsibilities required under ss. DHS 36.16 (2) and 36.19.
 
Essential Duties & Responsibilities:

-Provide ongoing psychosocial rehabilitative services and supportive psychotherapy, both in the home, community, and office, including ongoing assessment of the client’s mental illness/substance abuse symptoms and the response to treatment, symptom education to enable the client to identify his/her mental illness symptoms. Includes review of all CCS assessments and recovery/treatment plans to ensure proper completion and approval of service.

-Complete all program documentation, including ongoing monitoring and provision of services within client files in accordance with Federal Medicaid guidelines for psychosocial rehabilitation services and crisis intervention services.

-Facilitate, compile, and collect information for a detailed assessment, mental status exam and treatment plan for each participant in the program.

-Communicate the need for and role of the recovery team, facilitate team meetings and document recommendations, changes, additions, etc.

-Coordinate, follow up and monitor activities that ensure the client receives needed services, recovery planning, supportive activities, as well as coordinate planning and long-term goals for the client with families and other agencies using a recovery-oriented, person-centered approach.

-Develop and maintain safety and crisis plans on all individuals under DHS 34.

-Teach behavioral symptom management techniques to alleviate and manage symptoms not reduced with medication, and assist other program staff in development and practice of these techniques to their use with clients, promoting personal growth and development by assisting the client to adapt to and cope with internal stressors.

-Provide and make arrangements for provision of rehabilitation services that include employment-related services provided in community-based settings to assess the effect of the client’s mental illness on employment and to develop and ongoing rehabilitation plan to enable the client to obtain and maintain a job.

-Provide social and recreational skill training, including supervised teaching activities and experiences provided individually and in groups.

-Provide activities of daily living and support services in community-based settings including individualized support, problem solving, training and supervision to assist the client to gain or utilize the skills.

-Provide financial management to case specific clients, including budgeting, monitoring individual checkbooks and general client accounts.

-Provide crisis intervention services, including risk assessments, safety planning, and coordination of services under Ch. 51.15, under DHS 34 as part of a rotation of all clinical staff. This also includes coordinating discharge plans for individuals in inpatient treatment facilities by working cooperatively with the Intervention Services Supervisor and Clinical Case Management Supervisor, as well as providing information to the court, including court testimony in certain situations.

-Locate, secure, and monitor services provided to clients by rehabilitation worker(s).

-Provide training within the agency or within the community on topics specific to agency services or on overall mental health or substance abuse issues. 
 
Work Requirements:
-Bachelor's Degree in Social Work, Psychology or related field.

-Continuing education credits as prescribed by the Wisconsin Administrative Code and Department of Regulation and Licensing.    

-1000 hours of clinically supervised work with children and adolescents preferred. 

-Requires ability to complete 40 hours of orientation training specific to DHS 36 and maintain annual training thereafter.

-Requires ability to complete 40 hours of orientation training specific to DHS 34 and maintain annual training thereafter.

-Training in Non-Violent Crisis Intervention.

-Requires valid Wisconsin Driver’s license and adequate insurance as required by the County.
